Walled Lake Consolidated School District (WLCSD) offers a competitive benefits package that is comprehensive for eligible employees and their families. Attendees will need either their health insurance card, or a photocopy of, and government ID as well as this form completed.

      • FluBlok (protein-based, not egg-based)
      • Flu HD vaccine (which has been shown to have an improved response for those 65 and older)
    • Non-seasonal Vaccines
      • Shingles
      • Pneumonia
      • Tetanus
      • Whooping cough
    • Blood Pressure Screening
  • All Employees/Sub-contracted Employees of WLCSD with a valid health insurance card

  • Dependents with insurance are being asked to attend the flu clinic being held at the ESC along with WLCSD Employee.
