This file must be completed and turned in by a parent for any student needing to take medication during the school day
Distribution of Medication- Parent Letter
This letter explains the process to parents regarding administering medication at school.
Student Academic Integrity Contract
This file must be completed and turned in to the Smart offices or at Smart Start
Athletics physical form
This form must be completed and turned in to the main office prior to practicing in any sport

Please complete and submit to the Smart offices when requesting usage of any Smart classroom or space
